Starting September 29, 2025, we are implementing several exciting updates that we can’t wait to share with you. Below, you’ll find a detailed overview of what’s new and improved.

Enhanced Membership Plans

First, the structure of our membership plans will be changing for families who want direct access to our patient portal for messaging with Dr. Stevens about minor acute concerns or treatment plans. Our membership plan will ensure unlimited virtual or in-person sick visits during office hours, priority scheduling, and age-appropriate routine wellness visits throughout the year. Membership also includes an annual basic lab panel for children ages 3 and up, and discounts on other services and offerings. The membership pricing is straightforward as is described below! Please ask or patient care team about family plans when enrolling 3+ individuals under one household.

Membership Benefits:

• Unlimited Sick Visits: Virtual or in-person sick visits during office hours.

• Priority Scheduling: Get the appointments you need when you need them most.

• Age-Appropriate Wellness Visits: Regular checkups to ensure your child’s optimal health.

• Annual Lab Panels: A basic lab panel included for children aged 3 and up.

Membership Pricing:

• Children 0 - 35 Months: $90/month, $100 One Time Enrollment Fee

• Individuals 3 - 29 years: $60/month, $100 One Time Enrollment Fee

• Adults 30 - 59 year: $70/month, $175 One Time Enrollment Fee

• Adults 60+ year: $90/month, $175 One Time Enrollment Fee​

• All plans require a 12 month commitment

• Family plans for 3+ enrollments per household

Pay-As-You-Go Options for Nonmembers

If you prefer not to enroll in a membership, we’ve streamlined our pricing for individual visits:

Age 0-29 Wellness Visits:

• $150 per Well visit

• Acute Visits: $75 per visit

• Combined Wellness & Acute Visits: $175 (a $50 savings compared to separate billing)

• Age 

Age 30+

• $325 for the first initial well visit (45 minute comprehensive Eval)

• Billed at $300/hour for followups. (Typically 30 minutes for wellness and 15 minutes for acute)

Expanded Pediatric Functional Medicine Services

We’re thrilled to expand our personalized Functional Medicine offerings:

• Initial Consultation: $375

• Follow-Up Visits: $250/hour, prorated to time used

• In-House Phlebotomy Services:

  • $25 for the first tube

  • $10 for each additional tube

  • Maximum charge: $75 per visit

To ensure transparency, all labs can be coded for insurance reimbursement (though we cannot guarantee coverage). We also provide cash pay lab options to help avoid surprise costs.

Adult Functional Medicine Services

• $425 for Initial Consultation (75 Minutes)

• Followups billed at $300/hour and prorated accordingly

• In-House Phlebotomy Services:

  • $25 for the first tube

  • $10 for each additional tube

  • Maximum charge: $75 per visit

Updated Cancellation Policy

To better serve all families, we’ve revised our cancellation policy:

• 24 Business Hours’ Notice Required: For Monday appointments, cancellations must be made by 10 AM on the preceding Friday.

• Fees for Missed or Late-Canceled Appointments:

  • $50 for direct primary care visits

  • $100 for functional medicine appointments, regardless of appointment length

• Repeat Offenses: May require prepayment for future appointments.
